HOW DO I REQUEST THE RELEASE OF CLIENT HEALTHCARE INFORMATION?
Follow the prompts below. We will receive your request and follow up with you on the next steps.
HOW LONG DOES IT TAKE TO PROCESS REQUESTS?
This depends of factors such as if your release of information form was filled out correctly. We try to process requests as quickly as possible.
HOW WILL MY RECORDS BE DELIVERED
If you are a client requesting your own records, they will be securely delivered through your Simple Practice portal.
For third-party requests, we use MedTunnel, a HIPAA-compliant platform that allows for secure and efficient record sharing. Once your records are ready, you will receive an email from MedTunnel notifying you of a secure message. Follow the link in the email to create your MedTunnel account and access the documents.
Please note: While MedTunnel does offer messaging capabilities, we recommend emailing us directly at records@radzomcounseling.com for the fastest response.

Cost
IS THERE A CHARGE FOR THE RELEASE OF CLIENT HEALTHCARE INFORMATION?
If your request requires time outside of a standard session (e.g. phone calls, emails, letters, etc) we will bill at the rate of $165.00 per hour (pro-rated and billed in five-minute segments).
